Syllabus: How Does Psychotherapy Produce Change?

Follow the distinctions among symptoms, functioning, personal goals and intermediate processes.

Why you’re reading it. Establish what an explanation of therapeutic change is supposed to explain.

A question to consider. For someone seeking help after a relationship breakdown, propose two outcomes that could change differently and explain why both would matter.

Why this reading

Improvement needs a definition before it can be evaluated. Cuijpers compares symptom change with patient priorities, functioning, quality of life, unwanted effects and costs. The paper makes the choice of outcome part of the intellectual problem.

Before you read

A target is something treatment aims to change; an outcome is what is measured or observed. A standardized measure permits comparison across people, while an individualized measure can capture a goal that matters especially to one person.

Read for

  1. Compare what patients, clinicians and funders might count as success. Where could their priorities differ?
  2. Distinguish a desired result from an intermediate process proposed to produce it.

Consider afterwards

  1. Construct a case in which symptoms improve but an important personal goal remains unmet.
  2. What would an evaluation miss if it measured only benefits or only average change?

Editorial context

This is a narrative overview and argument about priorities. Its examples and selected meta-analyses are not an exhaustive comparison of treatments. Statements about the lack of established mechanisms reflect the author’s assessment in 2019 and the criteria he applies; they do not establish that no mechanism can be understood. Patient priority is also a value judgment to examine.
