Reading focus
Syllabus: How Should We Classify Psychopathology?
Examine the proposed treatment of essential features, normal variation and nearby diagnoses.
Why you’re reading it. Connect the previous unit’s purposes to the design of usable guidance.
A question to consider. How could replacing an exact threshold with a contextual judgment improve usefulness while creating a new reliability problem?
Why this reading
Diagnostic guidance must turn general concepts into decisions about particular presentations. This development report explains how an international classification project organized descriptions, boundaries and contextual qualifications around practical aims.
Before you read
ICD is the World Health Organization’s classification system. A diagnostic guideline can specify essential features without imposing a rigid numerical threshold for every judgment. Reliability, usefulness and validity remain different evaluation targets.
Read for
- Trace how the proposed format handles normal variation, neighboring diagnoses, development and culture.
- Separate the developers’ aims from evidence that their design achieves those aims.
Consider afterwards
- What could be gained and lost by replacing an exact threshold with clinical judgment?
- Design a comparison that could reveal whether a guideline makes an important distinction clearer.
Editorial context
Written by participants in ICD-11 development, this 2015 report describes plans and work in progress. It is not the current manual or an independent evaluation of outcomes. WHO subsequently published the Clinical Descriptions and Diagnostic Requirements in 2024.
