Syllabus: How Do Psychotic Experiences Develop and Persist?
Trace which proposed connections have direct human evidence and which depend on extrapolation or separate studies.
Why you’re reading it. This develops the biological question while keeping the earlier psychological account in view.
A question to consider. Choose one social-to-biological connection: what measurements and comparison would distinguish mediation from two parallel consequences of adversity?
Why this reading
An integrated explanation must connect levels, not merely mention them. Howes and Murray propose links among developmental vulnerability, social adversity, dopamine signaling and interpretations of experience. The paper lets readers examine the evidence for individual connections and the stronger claim that they form a reinforcing causal process.
Before you read
Dopamine is a signaling molecule with multiple functions. Presynaptic synthesis and release concern the sending side of a neural connection; receptor availability is a different measurement. Salience concerns what attracts significance or attention. Sensitization means a response grows after repeated exposure. None of these terms by itself defines a psychiatric diagnosis.
Read for
- Trace the proposed sequence from developmental and social conditions to signaling and interpretation. Mark where evidence comes from humans, animals or theoretical inference.
- Distinguish support for an individual connection from evidence for the complete feedback loop. What would observing all variables in one longitudinal study add?
- Compare the model’s account of episodes with its acknowledged uncertainty about persistent negative symptoms and cognitive difficulties.
Consider afterwards
- A social exposure predicts symptoms and a separate study finds altered dopamine signaling. What would be needed to demonstrate that the signaling change mediates the social association?
- Design a result that would favor a competing pathway while preserving some of the model’s observed associations.
Editorial context
This 2014 synthesis is a proposed integration, with several human links and dynamic changes explicitly identified as needing further evidence. Its genetic and treatment discussion reflects its period. Later large genomic work implicates broad neuronal and synaptic biology; it does not uniquely establish this causal chain. The statement that all available antipsychotics share the older mechanism is now historical: FDA records document a muscarinic treatment approved in 2024. Neither observation settles the full explanation of psychotic experiences.
