Reading focus

Syllabus: Why Do Responses to Trauma Follow Different Paths?

Compare the primary intrusion outcome with the broader prevention claim and the proposed memory explanation.

Why you’re reading it. Finish by separating a randomized package comparison from proof of mechanism or long-term recovery.

A question to consider. Propose a follow-up study with one comparison that tests the mechanism and one outcome that tests whether everyday functioning improves.

Why this reading

An explanatory theory can inspire an intervention that targets a specific experience. This trial makes it possible to examine the distance between changing an outcome, identifying a mechanism, and preventing a broader disorder.

Before you read

A proof-of-concept trial tests an initial prediction under bounded conditions. A primary outcome is the prespecified main endpoint. An intervention package can differ from its control in several ways, even when allocation is randomized.

Read for

  1. Compare every ingredient of the intervention and control, including the reminder procedure, and distinguish blinded from unblinded assessment.
  2. Keep the first-week intrusion diary separate from other symptoms and later outcomes.

Consider afterwards

  1. What additional comparison and follow-up would be needed to test the proposed memory mechanism and the separate claim of preventing PTSD?

Editorial context

Seventy-one emergency-department patients were randomized after motor-vehicle accidents. Fewer first-week diary intrusions do not establish prevention of PTSD or isolate a memory mechanism. The linked published commentary challenges the prevention framing. A later small exploratory pilot used a different control and reported wide uncertainty. These are selected follow-ups, not a comprehensive assessment of current treatment evidence. Issue year: 2018; first online: 2017.
